Public Health Solutions is the lead entity for the NY Medicaid 1115 waiver Social Care Network (SCN) in Queens, Brooklyn, and Manhattan. Through the SCN, eligible Medicaid members who are pregnant or postpartum through one year may be able to receive support for social needs related to food, transportation, housing, and care management. This can include refrigeration for the storage of breastmilk.
